If you are a UF student insured under ICS, and if you can not get a satisfactory claim from them, you may need some assistance from Student legal service, which is free for all full-time students. ICS will pay what they should pay as soon as they receive a letter from the lawyer. In my case, they paid 100% of the charges from the infirmary.
Based on my experience, ICS will give you only partial coverage and numerous excuses no matter how many times you write or call them. Don't be frustrated, just call the lawyer's office and they will help you out.
